I don't believe they would stack as they both inflict the same "Inhibit TP" effect. It also would've been 45% since Relic head increases Penance's Inhibit TP by 10% with max merits.

I haven't gotten a "Plague wears off" message the couple times I've tried Shijin Spiral, but I also don't Shijin Spiral in macc gear.
