Try playing COD MW2 on Xbox and see the hacks. Unless you hack you won't survive 10 seconds. Your screen will have strange text.

As for how, iirc they use a thumb drive to download the hack.

I'm sure you can buy one on eBay. I collect retro games and I still want to find a minty Game Genie for nostalgia sake including the original code book. I remember being like 12 and renting one.
