Make sure your graphics card is installed properly.

there is a drop down box where you can pick/choose what expansion to install. in there is the option to install the direct X 3d
choose that and that only and click install

Not 100% sure... If you're on PC (windows 8/10) , Control Panel > Programs > Turn Windows Features On or Off > Legacy Components > Tick DirectPlay.

Again, no. You do not need to install directx 8.1 from the FFXI installer.

the worst part is I can't remember the nvidia specific thing you had to do....
you had to go into the playonline files and there is a config or setup file with some boxes to tweak...
/sound of filing boxes being thrown around my brain

Oh also just make sure the driver is up to date, create a restore point first though.
Start -> All Programs -> PlayOnline -> PlayOnline Viewer -> PlayOnline Viewer Config
