In the 1970s a French man took pictures of himself cutting off parts of his penis with a knife and put them on display. They called that art. I don't think that's an excuse for somebody who clearly has issues. The guy/girl/thing can still express himself but be professional.

If I were a doctor, I wouldn't come to work and to my patients with a brief case, aviators, and a suit.
